How old are you? Who else will be living with the dog, and what are their ages?

What are you interested in pursuing with your poodle? Are you interested in a conformation prospect, a service dog prospect, a therapy dog prospect, an emotional support animal, a 'for fun' sports prospect, or a serious sport prospect? Or are you looking for a companion/pet?

Why a poodle? Do you have experience with the breed and their coat care? Does anyone in your household have dog allergies?

Do you have a groomer picked out locally? How often do you plan to take your dog to the groomer?

Do you have a vet picked out? If you already have pets, please include the name and phone number of your vet so I can call them. If you do not already have pets, understand that I require you to have picked a vet before the puppy goes home.

What are your feelings about vaccinations and standard wellness care and parasite preventions in dogs?

At what age would you plan to spay/neuter, if the dog is not a conformation prospect?

Are you willing to pursue formal training? I highly recommend at least puppy and basic obedience classes, if only for socialization. What other socialization plans do you have?

What other pets are in the household? If there are other dogs, are they intact or altered? Will they get along with a puppy; if not, what are your plans?

What are your past experiences owning pets?

What's your living situation like? Do you live in a house or an apartment? Do you rent or own? Do you have a yard? If so, describe the fence.

What type of flooring do you have, and do you have stairs?

What are your plans to exercise your pup?

Where will your dog sleep? What are your plans for the dog when you are working, or out running errands?
